{"id":77277,"date":"2024-05-01T08:00:26","date_gmt":"2024-05-01T12:00:26","guid":{"rendered":"https:\/\/www.policechiefmagazine.org\/?p=77277"},"modified":"2025-02-06T10:17:46","modified_gmt":"2025-02-06T15:17:46","slug":"promoting-comprehensive-wellness","status":"publish","type":"post","link":"https:\/\/www.policechiefmagazine.org\/promoting-comprehensive-wellness\/","title":{"rendered":"Promoting Comprehensive Wellness"},"content":{"rendered":"<p> <\/p>\n<p>The Military Police of the State of S\u00e3o Paulo (PMESP), Brazil, plays a crucial role in a vast territorial area of 248,219 sq. km (approximately 95,838 sq. miles), serving a significant population of 44 million inhabitants.\u00a0The force comprises approximately 80,000 active-duty PMESP officers spread across 645 municipalities, with an additional 70,000 veterans. The focus on mental health is a strategic objective of the institution, aiming to enhance actions for the protection of the physical and psychological well-being of PMESP officers, with a special emphasis on strengthening the mental health system.<\/p>\n<p>To address mental health needs, the Center for Psychological and Social Care (CAPS) was established in 1985, with the development of clinical psychology in the PMESP in 1990, in response to internal and societal demands. On May 23, 1994, clinical psychology services for officers and, in specific situations, their family members, were officially instituted in the PMESP.<\/p>\n<p>Currently, CAPS plays an important role in this context, striving to meet the needs of the force. The service comprises more than 100 psychologists distributed across 41 Psychosocial Care Centers (NAPS) covering the entire state territory. These centers are strategically located in macro-regions to decentralize the service and provide access to all officers.<\/p>\n<p>One of the tools utilized for mental health care is psychotherapy. It is believed that engaging in therapeutic settings provides PMESP officers with the opportunity to promote self-awareness, relieve tension from conflicts or stressful situations, develop emotional intelligence, improve personal and professional relationships, and overcome conflicts, promoting quality of life as a part of this prevention.<\/p>\n<p>During the COVID-19 pandemic, telepsychology was adopted as an innovative tool to ensure the continuity of mental health care for PMESP officers. The CAPS provided remote clinical psychology services during this period, which proved to be an effective solution to ensure that mental health care was not interrupted. Surprisingly, post-pandemic, telepsychology has become an important alternative for patients, driven by various reasons observed during practice.<\/p>\n<h3><span style=\"color: #9a1b22;\"><strong>Benefits of Telepsychology<\/strong><\/span><\/h3>\n<p>The implementation of telepsychology for the PMESP has brought lasting benefits beyond the pandemic, including the following:<\/p>\n<p><strong>Efficiency:<\/strong>\u00a0Telepsychology provides greater speed in service, allowing police officers to access mental health services more quickly, which is crucial for addressing urgent issues and preventing crises.<\/p>\n<p><strong>Accessibility:<\/strong>\u00a0Eliminating geographical barriers and travel difficulties makes psychotherapy more readily accessible to all officers, regardless of their location. This is particularly relevant in an institution that operates over such a vast territory.<\/p>\n<p><strong>Flexibility:<\/strong>\u00a0Offering adaptable schedules to the individual needs of officers contributes to the effectiveness of treatment. This recognizes the complexity of the routines of police professionals, enabling them to seek help without compromising their operational responsibilities.<\/p>\n<p><strong>Cost-effectiveness:<\/strong>\u00a0Reducing costs associated with physical travel to in-person sessions not only benefits officers but also optimizes resources, enabling a more efficient investment in promoting mental health within the organization.<\/p>\n<p><strong>Democratization:<\/strong>\u00a0By making psychotherapy available remotely, telepsychology democratizes access to mental health care, reaching a broader audience within the PMESP. This contributes to the development of a healthier and more resilient organizational culture.<\/p>\n<p>These benefits reinforce the ongoing importance of telepsychology as an integral part of mental health care in the PMESP, promoting a comprehensive approach tailored to constantly evolving demands.<\/p>\n<p>In 2022, a study on the offered services was initiated, and researchers found that the number of psychologists involved in various activities related to universal, selective, and indicated mental health prevention was insufficient to meet the demand for clinical care. Consequently, in late August 2023, a company was hired to carry out telepsychology activities, increasing the number of mental health professionals available for psychological clinical care, thereby favoring the expansion of service provision.<\/p>\n<p>This service is entirely free for PMESP officers, whether active-duty or veterans, with no associated fees or requirements for affiliation with any institution. To encourage utilization, it was decided not to mandate any prior referral from a doctor or other health care professional. The intention is for the PMESP officer to feel free to seek assistance without any constraints.<\/p>\n<table class=\"alignleft\" style=\"width: 404px; border-collapse: collapse; background-color: #ffffff; border-color: #ffffff; border-style: solid;\" border=\"1\" cellspacing=\"2\" cellpadding=\"2\">\n<tbody>\n<tr>\n<td style=\"width: 100%;\">\n<h5>Figure 1:\u00a0Increase in Appointments after Telepsychology Became Available<\/h5>\n<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 100%;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-77280\" src=\"https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/Figure-1-INCREASE-IN-APPOINTMENTS-300x203.png\" alt=\"A graph showing the 220 percent increase from the last quarter of 2022 (2,813 appointments) after implementing telepsychology in the last quarter of 2023 (6,214 appointments).\" width=\"400\" height=\"270\" srcset=\"https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/Figure-1-INCREASE-IN-APPOINTMENTS-300x203.png 300w, https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/Figure-1-INCREASE-IN-APPOINTMENTS-768x519.png 768w, https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/Figure-1-INCREASE-IN-APPOINTMENTS.png 987w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>After a quarter of operation of the expanded service, an increase in the number of patients and the quantity of psychotherapy sessions conducted was observed compared to the same period in 2022.<\/p>\n<p>In observation, during the last quarter of 2022, there were 2,813 appointments, whereas with the implementation of the telepsychology service in the last quarter of 2023, 6,214 appointments were provided, marking a remarkable increase of 220 percent compared to the previous period.<\/p>\n<p>Another significant outcome is the number of individuals who have attended at least one session, totaling 1,080 people.<\/p>\n<p>Additionally, feedback from users of the system was noted. During the first quarter of the service\u2019s operation, there were 2,313 responses in the evaluation of the platform\u2019s performance (NPS Scale), with scores ranging from 0 to 10. Of these, 1,979 respondents gave a perfect score of 10, and 203 respondents gave a score of 9. These results suggest that the users are satisfied with the service.<\/p>\n<p>These statistics underscore the positive impact of the telepsychology service, both in terms of increased accessibility and user satisfaction. The substantial rise in the number of appointments reflects the effectiveness of telepsychology in meeting the mental health needs of PMESP officers. The favorable feedback further supports the notion that the service is well received and aligns with the objective of promoting a healthier and more resilient organizational culture within the PMESP.<\/p>\n<h3><span style=\"color: #9a1b22;\"><strong>Tools and Care Initiatives<\/strong><\/span><\/h3>\n<p>However, merely offering more services is not sufficient for increasing mental health care. There are challenges related to the care of police officers that are universal across the profession:<\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul style=\"list-style-type: square; font-size: 18px;\">\n<li><strong>Fear of Exposure and Distrust of the Service:<\/strong>\u00a0The fear that the content discussed within sessions may be used by the institution to the detriment of the officer\u2019s career or may lead to being stigmatized as unwell.<\/li>\n<li><strong>Culture of independence:<\/strong>\u00a0The belief that police officers do not need help and should find ways to deal with their anxieties and reflections on their own.<\/li>\n<li><strong>Lack of knowledge about therapy:<\/strong>\u00a0Limited understanding of what therapy entails and what is addressed within the therapeutic setting.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>These concerns were brought to the attention of psychologists working at the CAPS, and the service aims to mitigate these feelings in various ways.<\/p>\n<p>It became evident that, in addition to being able to access the service, individuals must trust the work and engage with the care offered. Therefore, alongside an extensive campaign on how to use telepsychology, various care segments were developed that would subsequently contribute to an active search for the service.<\/p>\n<p>Every month, a visual campaign is launched with informative pieces on various topics related to personal care and mental health. These pieces not only provide knowledge to the police officers who see it, but also include contact information to access the available services.<\/p>\n<p>As part of the ongoing commitment to health promotion, the Emotional Management workshop was developed. The workshop aims to sensitize PMESP officers to the importance of self-awareness, support networks, and understanding psychosocial processes. The workshop not only provides resources and reflections to enhance mental health but also supports stress management, healthy interpersonal relationships, and illness prevention.<\/p>\n<p>Existing programs were also utilized to broaden the potential for care. The Career Closure Sensitization Program seeks to raise awareness and prepare professionals for the changes that will occur in their future lives upon transfer to reserve status. At the end of the program, participants are referred to psychotherapy if necessary.<\/p>\n<p>The Monitoring and Support Program for Military Police Officers is designed to assess the psychosocial conditions of PMESP officers involved in potentially traumatic situations, ensuring their physical and mental well-being. It addresses any emerging needs during the process and facilitates officers\u2019 smooth return to professional activities. After the initial evaluation, participants engage in a psycho-emotional development stage, where in-person or online clinical psychology sessions are offered if needed.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-medium wp-image-77286\" src=\"https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/733A0237-ai-copy-300x166.jpg\" alt=\"\" width=\"300\" height=\"166\" srcset=\"https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/733A0237-ai-copy-300x166.jpg 300w, https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/733A0237-ai-copy-768x424.jpg 768w, https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/733A0237-ai-copy.jpg 960w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/>In addition to providing mental health support, these initiatives are also intended to debunk unfounded beliefs regarding psychological and social assistance, which often act as barriers to seeking mental health services. These activities have proven to be effective instruments in disseminating the practices of CAPS, promoting the engagement of police officers in mental health care, and fostering a leadership culture that supports their development.<\/p>\n<p>By implementing these initiatives, the aim is to effectively strengthen the culture of mental health care, reduce stigmas, and build a solid foundation for the emotional well-being of PMESP officers.<\/p>\n<h3><span style=\"color: #9a1b22;\"><strong>Mental Health Policies<\/strong><\/span><\/h3>\n<p>The increased demand for clinical psychology services has enabled the formulation of broader guidelines to understand who seeks mental health services and when they do so, facilitating interactions with the public and mental health planning.<\/p>\n<p>Understanding the engaged audience for psychotherapy activities leads to an analysis that is the result of cross-referencing various data from monitoring programs, contributing to the construction of a more assertive and effective mental health policy. For example, an analysis may focus on identifying whether the risk factors associated with PMESP officers who die by suicide are present in officers seeking mental health help. If not, teams can study the phenomena and devise strategies, incorporating existing programs or developing new forms of care to strengthen the protective framework.<\/p>\n<p>Additionally, understanding the reasons for seeking the service allows for the identification of commonalities among system users, translating into activities tailored for specific care needs, such as new workshops, group activities, and more. In general, research suggests that identifying specific characteristics of subgroups and applying a combination of strategies, particularly prevention strategies to address risks and foster protective factors, is highly recommended.<\/p>\n<p>Continuous analysis, ongoing adaptations, and future strategies are essential to maintain the effectiveness of mental health initiatives in the PMESP. Continuous understanding of officers\u2019 needs and a readiness to adjust programs according to changing circumstances ensure more comprehensive and adaptable care is aligned with the evolving reality.<\/p>\n<h3><span style=\"color: #9a1b22;\"><strong>Continuous Adaptations and Future Strategies<\/strong><\/span><\/h3>\n<p>The commitment of the PMESP to the mental health of its officers is evident through the successful implementation of telepsychology and comprehensive initiatives. The substantial increase in demand for psychotherapy services reflects not only the service\u2019s effectiveness but also the urgent need for mental health care in the organization.<\/p>\n<p>Currently, there is a proposal for new legislation on the Mental Health System, aiming to establish a support network that facilitates the development of activities for the care of PMESP officers in various axes of biopsychosocial and spiritual health, as well as occupational health and safety.<\/p>\n<p>The updated regulations aim to bring changes to mental health policies in recent years, providing comprehensive, integrated, and responsive services and implementing strategies for mental health promotion and prevention. The new proposal intends to integrate various physical and social health care entities within the PMESP into mental health services, recognizing the significant impact of security-related activities on the lives, families, and careers of its members. The interaction with professionals beyond psychologists is seen as the most appropriate approach, promoting comprehensiveness and care, facilitating referrals, and more expansively addressing the wellness of officers. The strategies adopted so far show promising results, but it is imperative to continue adapting and innovating to overcome cultural and individual barriers.<\/p>\n<p>By addressing issues such as stigma, fear of exposure, and a lack of understanding about the benefits of therapy, the PMESP is paving the way for a healthier and more resilient organizational culture.
