{"id":79115,"date":"2024-09-01T08:00:15","date_gmt":"2024-09-01T12:00:15","guid":{"rendered":"https:\/\/www.policechiefmagazine.org\/?p=79115"},"modified":"2024-12-03T15:00:48","modified_gmt":"2024-12-03T20:00:48","slug":"making-mental-wellness-programs-effective-actionable-accountable","status":"publish","type":"post","link":"https:\/\/www.policechiefmagazine.org\/making-mental-wellness-programs-effective-actionable-accountable\/","title":{"rendered":"Focus on Officer Wellness: Making Mental Wellness Programs Effective, Actionable, and Accountable"},"content":{"rendered":"<p><span style=\"color: #672767;\"><strong>In a January 2024 Police Chief article, Dr. Sean Varano and his co-authors asserted that \u201cthe value of data is not a foreign concept to forward-leaning leaders. The concept of data-driven policymaking has made its way into most public sector organizations, and police agencies are no exception.\u201d<\/strong><\/span><\/p>\n<p>In describing the use of data in combating the overdose crisis, the authors concluded that, \u201cGood programs start with good data.\u201d This statement is generalizable to the mental wellness programs that have swelled in numbers during the past five years under various descriptions such as \u201cwellness checks,\u201d \u201ccheck-ins,\u201d and \u201cwellness visits.\u201d The growth of these programs led to the promulgation of guidelines from the IACP\u2019s Police Psychological Services Section\u2014and within these guidelines are these descriptions:<\/p>\n<ul>\n<li>\u201cThese visits aim to provide a convenient, confidential, and consistent way to promote early identification of possible mental health issues\u201d and to \u201cnormalize the experience of speaking to a mental health professional.\u201d<\/li>\n<li>\u201cWellness visits do not include a report of individually identifiable findings, rather than aggregated and anonymous data, to a third party.\u201d<\/li>\n<\/ul>\n<p>In 2019, The United States Department of Justice\u2019s Office of Community Oriented Policing Services published the results of their survey of various programs in <em>Law Enforcement Mental Health and Wellness Programs: Eleven Case Studies<\/em>. The publication cited the following issues in evaluating such programs:<\/p>\n<ol>\n<li style=\"list-style-type: none;\">\n<ol>\n<li style=\"list-style-type: none;\">\n<ol>\n<li>Most of the interviewees said their agencies do not track the number of cases or specifics of cases.<\/li>\n<li>While anecdotal evidence suggests that these programs are a success, a lack of research-based evidence makes it impossible to truly understand the impact of these programs.<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<p>To date, the data are lacking. No one knows if these programs are effective toward improving officer wellness generally\u2014and mental wellness specifically. Equally important is the lack of data to steer agencies to address the contributions of organizational stress and identifying resources, interventions, and psycho-educational support needed for their officers.<\/p>\n<h2><span style=\"color: #672767;\">Purposes of a Mental Wellness Visit<\/span><\/h2>\n<p><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-79135 alignleft\" src=\"https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/GettyImages-1355110895-300x158.jpg\" alt=\"\" width=\"410\" height=\"216\" srcset=\"https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/GettyImages-1355110895-300x158.jpg 300w, https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/GettyImages-1355110895-1024x540.jpg 1024w, https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/GettyImages-1355110895-768x405.jpg 768w, https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/GettyImages-1355110895-1536x810.jpg 1536w, https:\/\/www.policechiefmagazine.org\/wp-content\/uploads\/GettyImages-1355110895-2048x1080.jpg 2048w\" sizes=\"auto, (max-width: 410px) 100vw, 410px\" \/>There are some mental health professionals who focus on reducing stigma to seeing a mental health professional as the primary purpose. This viewpoint conceptualizes the mental wellness visit as only an educational opportunity, with the referring police agency provided results reduced to an officer either seen or not seen for a visit. In stark contrast, some states have legislated, under the guise of wellness, that officers be evaluated for psychological fitness. This approach seems to totally undermine the entire concept of improving officer mental health. So, what is the purpose? The authors of this article recommend that mental wellness visits, as a component of an overall department wellness program, are conducted at least every two years for the following purposes:<\/p>\n<ul>\n<li>An officer conducts a self-assessment of behavioral and mental health in key areas such as substance use, mood, organizational stress, trauma exposures, and continued commitment to service as a police officer.<\/li>\n<li>The health care professional integrates the self-assessment ratings during the mental wellness visits. Discussion about specific topics and referrals to potential resources are given to an officer for their use, as well as family members as needed. These interactions normalize sit-downs to discuss mental health and stress-coping tactics and provide an action plan during the interim prior to the next mental wellness visit.<\/li>\n<li>Individual officer responses that are acquired by the health care professional are confidential and protected information. The consent form that precedes the mental wellness visit specifies that only aggregate data are provided to the officer\u2019s employer\u2019s designated office on a semi-annual or annual basis. The latter may range from the human resources office to an occupational medicine program that administers wellness programs.<\/li>\n<\/ul>\n<blockquote><p><span style=\"color: #672767;\">Relying upon anecdotal reports reflects the missed opportunities for collaboration between police agencies and psychologist-researchers.<\/span><\/p><\/blockquote>\n<p>In the absence of data there is no ability to measure the effectiveness of mental wellness visits over time, the reduction in substance use, utilization of mental<\/p>\n<p>health services, sick leave utilization, psychotropic medication utilization, and many more indices. Only with metrics can a police agency evaluate needs for employee behavioral health services and organizational factors. Police executives are encouraged to implement mental wellness visits that incorporate the collection of aggregate data from which the program can be evaluated and action plans can be developed.<\/p>\n<h2><span style=\"color: #672767;\">Missed Opportunities<\/span><\/h2>\n<p>Police agencies should recognize that relying upon anecdotal reports reflects the missed opportunities for collaboration between police agencies and psychologist-researchers. Agencies can probably find a nearby university with doctoral programs in clinical psychology and criminal justice. These provide opportunities to collaborate with researchers. Further, collaboration with existing occupational health or behavioral health programs can provide an agency with actionable data about the effectiveness of the existing or proposed mental wellness visit programs. <span style=\"font-family: Webdings;\">d<\/span><\/p>\n<aside class=\"pullout pullout--wide alignleft\"><strong>IN MEMORIAM<\/strong><\/p>\n<p>We regret to inform you that one of the authors of this column, Dr. Stephen F.
